Any further information I require is set out hereunder..	Payment within 6 weeks of invoice
Further relevant information about my specific expertise on the subject matter in question (e.g. “This is a surgery that I am very familiar with and carry out regularly” or “I have published extensively in this area” [Narrative field max 500 characters]	I produce about 150 - 200 reports a year. I am happy to provide BoD, Causation and C&P reports. Happy to visit patients if necessary
